Dear Sixth Grade Parents,
Before your child begins seventh grade, state law requires that his/her vaccinations be updated. In addition to the vaccines your child has already received, seventh graders must have one dose of the meningococcal conjugate (MCV4) vaccine and a dose of the tetanus-diphtheria-acellular pertussis (Tdap) vaccine. The full vaccine requirements for seventh-grade students can be found on the DSHS Immunization Unit website at https://www.dshs.texas.gov/immunize/school/school-requirements.aspx.
To provide a convenient option for your child’s immunization update, FISD is hosting an Immunization Clinic on Monday, March 30, 2020, during the school day.
The cost of immunizations are as follows:
Currently Insured – No out-of-pocket expense (A copy of private insurance, Medicaid or Chips must be submitted)
No medical insurance - $5 per immunization
To take advantage of the Immunization Clinic, complete the DSHS Immunization Registry Form and the Aurora Concepts Consent Form and return them both to your child’s campus nurse by Friday, March 13, 2020. If you have medical insurance, please also include a copy of your insurance card. Parents are not required to be present at the time of immunization as long as the consent forms have been completed.
